Remove the function prototypes for ssl_cert_get0_next_certificate, ssl_set_default_md, tls1_shared_list,
dtls1_send_newsession_ticket, tls1_ctrl, and tls1_callback_ctrl, all of which are not defined.
It also changed the signature of the function pqueue_next to `pitem *pqueue_next(piterator *item)` in
pqueue.c, making it match the prototype in ssl_locl.h. (`piterator *` is equivalent to `pitem **`.)

The function pqueue_print is not exported and is never called. Therefore
we should delete it.

This was done by the following
find . -name '*.[ch]' | /tmp/pl
where /tmp/pl is the following three-line script:
print unless $. == 1 && m@/\* .*\.[ch] \*/@;
close ARGV if eof; # Close file to reset $.
And then some hand-editing of other files.

This is an internal facility, never documented, not for
public consumption. Move it into ssl (where it's only used
for DTLS).
I also made the typedef's for pqueue and pitem follow our style: they
name structures, not pointers.
