555cbb328e Collapse ssl3_state_st (s3) into ssl_st
With the removal of SSLv2, the s3 structure is always allocated, so
there is little point in having it be an allocated pointer. Collapse
the ssl3_state_st structure into ssl_st and fixup any references.

This should be faster than going through an indirection and due to
fewer allocations, but I'm not seeing any significant performance
improvement; it seems to be within the margin of error in timing.

5d67110173 Don't calculate the Finished MAC twice
In <= TLSv1.2 a Finished message always comes immediately after a CCS
except in the case of NPN where there is an additional message between
the CCS and Finished. Historically we always calculated the Finished MAC
when we processed the CCS. However to deal with NPN we also calculated it
when we receive the Finished message. Really this should only have been
done if we hand negotiated NPN.

This simplifies the code to only calculate the MAC when we receive the
Finished. In 1.1.1 we need to do it this way anyway because there is no
CCS (except in middlebox compat mode) in TLSv1.3.

Coincidentally, this commit also fixes the fact that no-nextprotoneg does
not currently work in master.

0e97f1e1a7 (Re)move some things from e_os.h
Remove GETPID_IS_MEANINGLESS and osslargused.

Move socket-related things to new file internal/sockets.h; this is now
only needed by four(!!!) files.  Compiles should be a bit faster.
Remove USE_SOCKETS ifdef's

270d65fa34 Always flush the BIO when we send any alert
At the moment we flush the write BIO if we send a fatal alert, but not a
warning one. This can mean the warning is never sent if we never do another
write and subsequently flush the BIO. Instead we should just always flush
after writing an alert.

49e7fe12ea Provide functions to write early data
We provide SSL_write_early() which *must* be called first on a connection
(prior to any other IO function including SSL_connect()/SSL_do_handshake()).
Also SSL_write_early_finish() which signals the end of early data.

d102d9df86 Implement write pipeline support in libssl
Use the new pipeline cipher capability to encrypt multiple records being
written out all in one go. Two new SSL/SSL_CTX parameters can be used to
control how this works: max_pipelines and split_send_fragment.

max_pipelines defines the maximum number of pipelines that can ever be used
in one go for a single connection. It must always be less than or equal to
SSL_MAX_PIPELINES (currently defined to be 32). By default only one
pipeline will be used (i.e. normal non-parallel operation).

split_send_fragment defines how data is split up into pipelines. The number
of pipelines used will be determined by the amount of data provided to the
SSL_write call divided by split_send_fragment. For example if
split_send_fragment is set to 2000 and max_pipelines is 4 then:
SSL_write called with 0-2000 bytes == 1 pipeline used
SSL_write called with 2001-4000 bytes == 2 pipelines used
SSL_write called with 4001-6000 bytes == 3 pipelines used
SSL_write_called with 6001+ bytes == 4 pipelines used

split_send_fragment must always be less than or equal to max_send_fragment.
By default it is set to be equal to max_send_fragment. This will mean that
the same number of records will always be created as would have been
created in the non-parallel case, although the data will be apportioned
differently. In the parallel case data will be spread equally between the
pipelines.

This was done by the following
        find . -name '*.[ch]' | /tmp/pl
where /tmp/pl is the following three-line script:
        print unless $. == 1 && m@/\* .*\.[ch] \*/@;
        close ARGV if eof; # Close file to reset $.

And then some hand-editing of other files.
