There are header files in crypto/ that are used by a number of crypto/
submodules. Move those to crypto/include/internal and adapt the
affected source code and Makefiles.
The header files that got moved are:
crypto/cryptolib.h
crypto/md32_common.h

For the various string-compare routines (strcmp, strcasecmp, str.*cmp)
use "strcmp()==0" instead of "!strcmp()"

Remove old M_ASN1_ macros and replace any occurences with the corresponding
function.
Remove d2i_ASN1_bytes, d2i_ASN1_SET, i2d_ASN1_SET: no longer used internally.

pointers (i2d, d2i, new, free). Change standard extensions
to support this.
Fix a warning in BN_mul(), bn_mul.c about uninitialised 'j'.
seemed like a good idea at the time... several hours later it was rather
obvious that these are used all over the place making the changes rather
extensive.
without -debug option to mk1mf.pl. Change _export to is_export (_export is
a reserved word under VC++). Add yucky function prototype function pointer
casts. Sanitise the included files in crypto/x509v3.
Also changed ssleay.exe target to openssl.exe
name, issuer and authority key id. Change the i2v function parameters
and add an extra 'crl' parameter in the X509V3_CTX structure: guess
what that's for :-) Fix to ASN1 macro which messed up
IMPLICIT tag and add f_enum.c which adds a2i, i2a for ENUMERATED.
