Move digest code into the relevant providers (fips, default, legacy).
The headers are temporarily moved to be internal, and will be moved
into providers after all external references are resolved. The deprecated
digest code can not be removed until EVP_PKEY (signing) is supported by
providers. EVP_MD data can also not yet be cleaned up for the same reasons.

Removed e_os.h from all bar three headers (apps/apps.h crypto/bio/bio_lcl.h and
ssl/ssl_locl.h).
Added e_os.h into the files that need it now.

Since NDEBUG is defined unconditionally on command line for release
builds, we can omit *_DEBUG options in favour of effective "all-on"
in debug builds exercised though CI.

This minimizes inter-block overhead. Performance gain naturally
varies from case to case, up to 10% was spotted so far. There is
one thing to recognize, given same circumstances gain would be
higher faster computational part is. Or in other words biggest
improvement coefficient would have been observed with assembly.

Don't have #error statements in header files, but instead wrap
the contents of that file in #ifndef OPENSSL_NO_xxx
This means it is now always safe to include the header file.
